How a Health Plan and Community Organizations Can Collaborate to Improve Health Outcomes

When Sarah became a member of Elevance Health’s Ohio Medicaid Plan, a screening test revealed she was at risk for health complications related to pregnancy and in need of support for health-related social needs. Her health plan case manager Lisa helped connect Sarah with resources for housing, food, transportation, employment and more, and set her up with the Salvation Army’s “Pathway of Hope” program. This 18-month long program connected Sarah to local community resources and support, including helping her enroll in a GED course. Sarah expressed her surprise and appreciation that a health insurance plan could support her in this way.
